Your Responsibilities
You are responsible for any co-payments or deductibles at the time of your visit. You are also responsible for any and all balances regardless if your insurance company pays.
We file insurance claims as a courtesy to our patients. You will receive an Explanation of Benefits (EOB) from your insurance provider that explains what services were covered and what was paid on them. We will send you an invoice that shows what your insurance company paid, any copayments made and any balance due.
Coverage of the services we provide varies by insurance company. Typically, services provided through our Snoring Institute are not covered by insurance. However, services provided through our Sleep Center may be, depending on the specifics of your policy.
You are also responsible for checking with your Medicare or Medicaid insurance provider to see if you need a referral to see a physician in our office
